A Brief Overview
Defines patient care problems and gives instructions to ensure timely and effective action and quality of care. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.
What you will do
- Demonstrates clinical expertise with regards to age specific populations, including neonates, pediatrics, adolescents, adults and geriatrics.
- Provides complete accurate documentation of each phone call and follow-up.
- Communicate clear, concise information both verbally and in writing to all team members.
- Assists interdisciplinary group in providing 24 hour, 7 days per week care.
- Demonstrates initiative in identifying opportunities for self-development and enhancement of professional competency.
- Understands, applies and supports Hospice, Alliance, and Methodist LeBonheur Healthcare policies, procedures and standards.
Education/Formal Training Requirements
- Required - Technical Degree or Diploma Nursing
Work Experience Requirements
- Preferred - Home care/hospice 1-3 years
Licenses and Certifications Requirements
- Required - Licensed Practical Nurse - Tennessee - Tennessee Board of Nursing
Knowledge, Skills and Abilities
- Work is performed off-site; must provide own transportation to deliver paperwork and attend meetings.
- Ability to understand complex written materials, such as patient records.
- Ability to communicate verbally with all levels of associates, management, physicians, and patients and their families.
- Ability to work without close supervision, and to exercise independent judgement in a professional area.
- Ability to work within an interdisciplinary environment as a member of a team dedicated to shared patient/family responsibilities.
- Ability to organize multiple tasks and projects and maintain control of work flow.

Physical Demands
- Ability to react quickly to multiple and emergency situations.
- Ability to read, write, and communicate verbally and in writing to other individuals.
- Ability to perform repetitive motions.
- Ability to stay calm during periods of high demand, remain responsive to patient/family needs, demonstrate follow-through of action taken, recommendations made.
