A Brief Overview
Works as part of the Home Health team to assess, plan, and treat patients with rehabilitative needs. Provides effective treatment plans in the patient's home under the Plan of Care. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.
What you will do
- Performs initial patient assessments and re-assessments.
- Provides effective treatment plans in accordance with the Plan of Care.
- Identifies patient specific learning needs and provides education as appropriate.
- Maintains reliable systems to document, track and monitor cases.
- Provides oversight for the treatment plans by contributing as part of a collaborative team effort, operating within scope of practice.
- Performs other job functions as assigned or requested.
Education Qualifications
- Master's Degree Speech Pathology
Experience Qualifications
- No prior work experience
Skills and Abilities
- Thorough knowledge of current theories and principles in field of specialty.
- Demonstrated ability to interact with peers, medical staff, patients/families and other constituents involved in patient care.
- Knowledgeable of the system through which the patient must move in order to obtain care.
- Must have excellent skills in verbal and written communication, problem solving, and conflict resolution.
- Must be able to organize home visit schedules and work without close supervision.
- Displays sensitivity, compassion, and respect for people of diverse cultures.

- Speech Pathologist Tennessee - Tennessee Board of Communications Disorders and Sciences
- Speech Pathologist Mississippi - Mississippi State Department of Health Professional Licensure Division
Supervision Provided by this Position
- There are no supervisory responsibilities assigned to this position.
Physical Demands
- The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
- Must have good balance and coordination.
- The physical requirements of this position are: medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently.
- The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
- Frequent non-invasive patient contact.
- Exposure to patient body fluids, inclement weather, temperature extremes, home environment risks such as safety, pets, fire, infection, etc.
- Ability to react quickly to emergency situations.
- Valid drivers’ license and current automobile liability insurance. Must be able to travel to patient’s residence for care delivery.
