A Brief Overview
Analyzes records in multiple formats, including paper and electronic, to ensure accuracy of demographic and statistical information, ensure presence of required documentation to meet DNV, CMS, and other agency requirements, and assure quality of scanned record images and accurate indexing in the electronic record. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.
What you will do
- Analyzes patient records for completeness of information according to guidelines established by Medical Staff Rules and Regulations, DNV, CMS, and other regulatory agencies.
- Obtains missing documents, re-scans documents of poor quality, makes corrections to the electronic medical record and places documents in appropriate order to meet required guidelines.
- Assists physicians and other clinicians with completion of deficiencies and delinquency related issues.
- Performs other job functions as assigned or requested.
Education/Formal Training Requirements
- High School Diploma or Equivalent
- Bachelor's Degree
Work Experience Requirements
- 1-3 years Health Information Management
Knowledge, Skills and Abilities
- Knowledge of DNV, CMS, and HIPAA standards and documentation requirements.
- Obtains charts and required documentation needed to facilitate coding.
- Exhibits full comprehension of multiple computer applications and serves as a resource for others.
- Provides information and responds to requests/questions from Patient Financial Services, Physician/Offices and others.
- Provides back up as needed to ensure timely processing of records.
- Maintains care, control and confidentiality of hospital medical records and retrieves medical records for authorized personnel as needed.
Supervision Provided by this Position
- May have some lead responsibilities to include training, orientation, work assignment and feedback to management regarding Associate performance.
- May demonstrate various tasks to students enrolled in healthcare programs.
Physical Demands
- The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
- Must have good balance and coordination.
- The physical requirements of this position are: light work - exerting up to 25 lbs. of force occasionally and/or up to 10 lbs. of force frequently.
- The Associate is required to have close visual acuity to perform an activity, such as preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
- The conditions to which the Associate will be subject in this position: The Associate is not substantially exposed to adverse environmental conditions; job functions are typically performed under conditions such as those found in general office or administrative work.
