Responsible for receiving and processing referrals on potential home care/hospice patients and coordination of data collection and entry. A Brief Overview
Responsible for receiving and processing referrals on potential home care/hospice patients and coordination of data collection and entry. Verifies and enters all physician orders, patient demographics, and insurance status. Models appropriate behavior as exemplified in The Methodist Mission, Vision and Values.

What you will do

  • Receives referrals for Home Health/Hospice Services.
  • Reviews clinical documentation; identifies Home Health and Hospice appropriate patients for admission.
  • Communicates with hospital Case Management to identify patient Home Health and/or Hospice needs.
  • Offers assistance to patients, patient’s families, customers, physicians, Home Health/Hospice agencies, and other referral sources.
  • Assures integrity of patient data and statistics.
  • Prepares correspondence and materials for Case Coordinators.
  • Performs other duties as assigned.


Education Qualifications

  • Technical Degree or Diploma Nursing

Experience Qualifications

  • 1-3 years administrative, customer service, or healthcare experience, preferably from a home health environment.

Skills and Abilities

  • Proven record of providing excellent customer service both internally and externally.
  • Knowledge of third-party payers.
  • Ability to evaluate problematic situations and be able to adapt, respond to, and/or notify/advise appropriate staff in order to resolve the situation/issue.
  • Ability to set priorities, coordinate multiple tasks, maintain workflow, and prepare written materials.
  • Ability to work without close supervision and to exercise independent judgment.
  • Ability to operate standard office equipment.
  • Skill and proficiency in oral and written communication including spelling, grammar, punctuation, and composition.
  • Must be proficient in the Microsoft Office products including Word, Excel, and Outlook.

  • Licensed Practical Nurse - Tennessee - Tennessee Board of Nursing
  • Licensed Practical Nurse - Mississippi - Mississippi Board of Nursing
  • Licensed Practical Nurse - Arkansas - Arkansas State Board of Nursing

Supervision Provided by this Position

  • There are no supervisory or lead responsibilities assigned to this position.

Physical Demands

  • The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
  • Must have good balance and coordination.
  • The physical requirements of this position are: light work - exerting up to 25 lbs. of force occasionally and/or up to 10 lbs. of force frequently.
  • The Associate is required to have close visual acuity to perform an activity, such as preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
  • The conditions to which the Associate will be subject in this position: The Associate is not substantially exposed to adverse environmental conditions; job functions are typically performed under conditions such as those found in general office or administrative work.
