A Brief Overview
Responsible for comprehensive diabetes education by teaching patients in a clinical setting about meal planning, diabetes medications, exercise, blood glucose monitoring, and integrating diabetes care into daily life. Plans, implements, and evaluates diabetes self-management education. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.

What you will do

  • Provides diabetes self-management education and training to individuals and groups in accordance with standards of professional practice for Certified Diabetes Educators.
  • Develops a prioritized holistic plan of care based on a comprehensive assessment, desired outcomes, and practice excellence.
  • Develops educational goals, learning objectives, and a plan for educational content and teaching methods.
  • Promotes a team approach to diabetes care through consultation and education of the clinical staff.
  • Delivers evidence Based/Best Practice and Quality Initiatives.
  • Participates in the planning, coordinating and providing diabetes-related health promotion activities.
  • Performs other job functions as assigned or requested.


Education Qualifications

  • Bachelor's Degree Nursing
  • Bachelor's Degree Dietetics
  • Bachelor's Degree Nutrition

Experience Qualifications

  • 1-3 years Clinical healthcare Must have at least two (2) years of clinical experience working with the diabetes population.

Skills and Abilities

  • Knowledgeable of current diabetes treatment management through a combination of professional experience, continuing education, study and mentorship.
  • Must be able to teach basic and advanced self-management skills to the patient and family.
  • Effectively communicates in writing and orally with patients/families and other health team members.
  • Ability to develop a collaborative, therapeutic alliance with patients.
  • Ability to operate and educate diabetes care medical equipment.
  • Individual and group teaching experience is desired, particularly in the areas of diabetes and insulin delivery techniques.
  • Skill and proficiency in applying highly technical principals, concepts, techniques and processes.
  • Analytical skills necessary to assess complex patient conditions effectively and to develop comprehensive plans of care.
  • Ability to synthesize multiple sources of information to assist in the resolution of human responses to complex problems.

  • Certified Diabetes Educator - National Certification Board for Diabetes Educators
  • Dietitian/Nutritionist Tennessee - Tennessee Board of Dietitians/Nutritionist Examiners
  • Dietitian/Nutritionist Mississippi - Mississippi State Department of Health
  • Registered Nurse Arkansas - Arkansas State Board of Nursing
  • Registered Nurse Mississippi - Mississippi Board of Nursing
  • Registered Nurse Tennessee - Tennessee Board of Nursing

Supervision Provided by this Position

  • There are no supervisory or lead responsibilities assigned to this position

Physical Demands

  • The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
  • Must have good balance and coordination.
  • The physical requirements of this position are: medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently.
  • The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
  • Frequent invasive and non-invasive patient contact.
  • Exposure to patient body fluids as well as exposure to hazardous or poisonous materials.
  • Ability to react quickly to emergency situations.
