The Case Management/PAS Assistant is responsible for the regulatory aspect of our IMM/GCOA/Moon forms and advance directives, maintaining up to date files for auditing, auditing our facility and other MLH facilities, and training of new associates on the regulatory issues of our IMM forms. A Brief Overview
The Case Management/PAS Assistant is responsible for the regulatory aspect of our IMM/GCOA/Moon forms and advance directives, maintaining up to date files for auditing, auditing our facility and other MLH facilities, and training of new associates on the regulatory issues of our IMM forms. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.

What you will do
  • Ensures that Important Message from Medicare (IMM) and Medicare Outpatient Observation Notice (Moon) Notices are signed dated and timed within 48 hours of patient admission.
  • Ensures Medicare and Medicare Advantage patients/partner in care have signed their IMM notice within 2 days of discharge or within four hours of their discharge departure from the hospital.
  • Runs all daily reports via Cerner and MoreCare, and prioritize patient visits based on anticipated discharge dates.
  • Maintains and Audit charts for MLH documentation standards regarding IMM notice.
  • Conducts daily chart review to ensure that all forms are signed, dated, timed, and returned to the patient’s record by end of each day.
  • Ensures all unsigned forms are mailed via Certified Mail within 2 day time window.
  • Performs other job functions as assigned or requested.

Education/Formal Training Requirements
  • High School Diploma or Equivalent

Work Experience Requirements
  • 1-3 years Healthcare or medical office environment

Knowledge, Skills and Abilities
  • Knowledge of medical terminology.
  • Strong verbal and written communication skills, including spelling and correct grammar accuracy.
  • Critical thinking and problem solving skills along with a level of attention to detail.
  • Ability to perform data collection.
  • Ability to organize tasks and maintain control of work flow.
  • Skill in applying appropriate behavior when greeting and assisting customers in person and by telephone.
  • Ability to operate standard office equipment.
  • Ability to use independent judgement to resolve problems and perform tasks with limited supervision.

Supervision Provided by this Position
  • There are no supervisory or lead responsibilities assigned to this job.

Physical Demands
  • Frequent non-invasive patient contact throughout shift.
  • Possible exposure to patient body fluids.
  • Ability to react quickly in emergency situations.
  • Ability to lift and carry up to 15 lbs. periodically throughout shift.
  • The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
  • Must have good balance and coordination.
  • The Associate is required to have close visual acuity to perform an activity, such as preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
  • The conditions to which the Associate will be subject in this position: The Associate is not substantially exposed to adverse environmental conditions; job functions are typically performed under conditions such as those found in general office or administrative work.
